Consultant Ophthalmology

Description

CPL UK are currently seeking an experienced Locum Consultant Ophthalmology to take up a long term post in Wales, starting ASAP.

This is a full time consultant post with a 1:5 on call. On-calls are 9am - 10pm and they do 7 days on-call in a block, which runs Friday to Thursday - the frequency is 1 in 5 but the locums may be asked to pick up more.

• Right to Work in the UK (passport/visa)

• DBS Certificate

• GMC Certificate

• CV and 2 x NHS References - two dated with 12 months, must cover last three years of employment

• Annual Mandatory Training Certs including Life Support Cert

• 2 x Proof of Address – dated within 3 months

• Most recent Appraisal pack

• Immunisation/bloods reports - We require a copy of these so that we may obtain an Occupational Health Clearance for you.
