Pay Rates (Hourly) : Days: £26 Saturdays/Nights: £30.70 Sunday/Bank Holidays: £34

All candidates must have a minimum of 6 months paid experience within a similar role from the last 3 years
 
Duties:                                               
Complete patient assessment and determine their clinical management needs
Manage patient’s care
Increase patient involvement in valuable activities in the community
Administration of medication
Ensure accurate records are set up and maintained for all contacts
